Gastroenterology Update 2024
by Dr Marius van Rijnsoever
About Dr Marius
Born in Holland and trained in Australia / New Zealand, I am an advanced interventional endoscopist specialising in gastroscopy, colonoscopy, endoscopic mucosal resection, endoscopic ultrasound and ERCP. My clinics are at Waitemata Gastroenterology and I perform endoscopy at WDHB and Waitemata Endoscopy.
New location and fast clinic letters
My main clinic location is now 7 Anzac St, Takapuna, the new convenient clinic site for Waitemata Gastroenterology.
Clinic letters will now be available to you and the patient within 24 hours, with the help of AI transcription software and longer clinic slots. I also have clinics at Waitemata Endoscopy in Henderson.
Pancreas cancer prevention trial
With the help of a Gut Cancer Foundation grant, the first pancreatic cancer prevention trial in Australasia is starting at WDHB by injecting gemcitabine/paclitaxel chemotherapy into high-risk pancreatic cysts.
Please contact me if you have patients with IPMN pancreatic cysts greater than 3 cm or worrying features to see if they are eligible.
What is in a name? NAFLD no longer
Non-alcoholic fatty liver disease (NAFLD) has been renamed metabolic dysfunction-associated steatotic liver disease (MASLD). The more severe inflammatory/scarring subgroup previously called NASH is now called metabolic dysfunction-associated steatohepatitis (MASH).
Is it good to have a better definition, a confusing change, or just a more patient-friendly term?
Change in hepatitis B terms
Hepatitis B terminology has also changed to avoid phrases such as “inactive carrier phase”, as liver damage can still occur in this phase. The natural history graph and terminology table are shown below.
GP Gastro Reference Sheet
Clinical risk calculators

REAL-B > 4: Screen. REACH-B > 9: Screen and consider antiviral therapy. Screening involves six-monthly AFP and ultrasound.
